Almost perfect!

This is almost exactly what I was looking for. I checked out a few other apps, but they seemed to lack the following features that I wanted:

-Customize fields.
-Multiple entries per day.
-Automatic time stamps.
-Time stamped notes.
-Easy data entry.

What I would like to see added:

-Yes/No fields.
-More than 10 scales per category.
-Tick marks or numbers on the scale.
-Ability to set default values on scales.

Still a great app and will continue to use it!
